| Consultation at our facility requires
referral by a health care provider. Our office staff is available Monday
through Friday, 8 am - 5 pm to assist with the referral process.
We ask that referring providers forward copies of office notes, operative reports, and results of radiographic and diagnostic studies when available.
A comprehensive pain history questionnaire will be sent to all patients prior to their appointment. |
